How much time is generally between the conclusion of phone interviews and the invitation for campus interviews?
A week? Two? Months?

I've been wondering the exact same thing, so I'm hoping someone can chime in here. I suspect it depends on a lot of things, like how long the phone interview list was, how often the committee meets, etc.
I know of one search this year that is doing phone interviews in mid-November and then campus interviews in late January!

I'm sure it depends on the school. I was invited to interview about a week after my phone interview (although it was slightly less than a week after the search committee concluded their phone interviews, I believe). The on-campus interview was scheduled for about two weeks after that time.
I have no idea whether that is typical, though.

Well, I sure hope it is typical. I would love to hear something this week about the results of the phone interview I had last week.

I was invited to campus a week after the phone interview, and the visit was scheduled about 2 weeks after I received the invite, so it seems my experience is similar to that of anon.

Come on... there is no reason for anyone to worry about stuff like this or try to devine the answer by triangulating other people's experiences on an anonymous blog. Just email whoever it was who interviewed you, or the admin who set up the phone interview, and ask!
They asked you to interview and so they owe you the courtesy of letting you know when a decision will be made. You should be sending short follow-up "thanks for interviewing me" emails after the phone interview, and that is a good time to also ask about the time line for the search.

I did that after a phone interview last week and received no response. I feel like it would be inappropriate to try again. Thoughts?

I feel as if sending a second email is probably not a good idea.

The SC will contact you if they want to schedule an on-campus interview. If you didn't make that cut, you'll be held as a backup, but there's little chance they will tell you that. Don't go begging for an early rejection, just let the process play itself out.
Tip for future phone interviews: always ask about the timeline. Experienced interviewers will tell you near the end of the conversation, but they if you omit this information it is certainly appropriate for you to bring it up. "Could you tell me about your timeline for moving forward? When do you expect to invite candidates to campus?"

I would not send a second email. Honestly, I wouldn't ask in the first! I always ask at the end of a phone interview - and the SC is usually somewhat vague, like "soon" or "in the next few weeks." And that is fine. I wouldn't keep asking because I wouldn't want to come across as desperate. However, if you have an offer somewhere else, and still are interested in the school - then perhaps you could ask again and tell them you have a timeline.

"Could you tell me about your timeline for moving forward? When do you expect to invite candidates to campus?" The interview ended pretty abruptly, we were running a few minutes over. I didn't get to ask. Needless to say, I'm pretty anxious to find out whether they want me to visit.

If you already emailed, then there isn't much else you can do. I wouldn't email again. Sorry! I know the waiting sucks. You can try to post here and ask if anyone has any insider info.

Sux but the best thing you can do is wait. Trust me, you don't want to come off as desperate (or annoying - yes, personal quirks can get you excluded). From my experience, the amount of time between phone interviews and campus interviews range between a week to two months (SC was waiting for a SC member to come back from vacation to decide on finalists).

Geez, everybody. I never said I was going to send another email.
It wasn't obnoxious to ask in my original thank-you email, though, right?

Nope, I don't think it was obnoxious to do so.
