How good is your gut feeling?

After interviews, how good is your gut feeling with regards to whether or not you got the offer?

I am asking because apparently my gut is terrible at discerning these things. Jobs I thought I was surely going to get an offer passed me over, and vice-versa.

My gut has often been correct in predicting offers, but I'd argue it's more important for your brain to focus on red flags. Were there things that took place during the interview process that should make you consider rejecting an offer if your gut was right?
