When a job ad asks for "Please send a curriculum vita, statement of teaching philosophy, research and teaching interests, etc"
Do you include these three things in one document (the cover letter)? Or do you send three different things? (note how statement is in the singular here).

That's a poorly worded job ad. Based on my experience (having been on both sides), search committees are forgiving as long as your application materials CLEARLY include all of the information they request. Of course, it all depends on who's on the search committee. I can see certain individuals thinking (and even saying out loud to other search committee members) - "What? This candidate can't read and follow instructions?"
